Things to know....

 

For any activities outside of school (Yes, as long as you are out of the school gate), there is a standard operational procedure (SOP) to adhere to. The sequences of the SOP are shown in the flow chart below.

 

 

 

3 weeks before activity:

 

Prepare and submit the following documents for approval by Mr Tan HL (1st vet)  and Principal (final vet):

  1. RAMS (To include RAMS from vendor if activity involves vendor.)

  2. 1 copy of consent form signed by CCA HOD with school stamp. This will be the same consent form that is subsequently given out to the students.

  3. List of students involved (preliminary list). 

  4. List of students' medical details and emergency contacts (requirement subjected based on nature of activity)

1-2 week before date of activity:

 

Consent form to be given out to students.

1 week before date of activity:

 

(Optional based on requirement) Bus booking form to be filled in and submit to Salinah.

Actual day of activity:

 

  1. Ensure all consent forms are duly signed and returned.

  2. If form is not signed, call parent/guardian of student to inform/ensure that parent/guardian was informed of the activity and permission had been granted to participate in the activity.

  3. A SMS reply from parent/guardian indicating his/her approval is required after the phone conversation as a means to document the approval.

Before commencement of activity:

 

  1. Briefing of activity objective.

  2. Briefing of safety measures such as evacuation plan.

  3. Verbal and visual check on health status of all students.

  4. Inform office (Salinah) to update (weather) CAT status if activity is outdoor.

  5. Ensure that all documents which had been approved are being brought along for activity.

Commencement of activity

Post activity:

Submission of LEAPS Form for participants. Only for the following events:

  1. Competition organised by HQ such as BB Blaze & Character Quest.

  2. BB Share-A-Gift.

  3. Other events representing school.
